Employee killed in fall from A-frame ladder

Employee #1 was on an 8 ft A-frame stepladder looking for a ground fault on a 24-volt fire alarm system when, for some reason, both he and the ladder fell. Employee #1 suffered a head injury and was transported to the hospital, where he died on September 19, 2000.
