Employee struck and killed when chain backlashes

Employee #1 was unlashing trailers in the hole of a roll on/roll off (ro-ro) vessel. He was unable to undo one chain at the rear of a trailer and decided to wait for the utility vehicle to return to lift the end of the trailer, loosening the tight chain. In the meantime, he apparently went to undo another trailer. Employee #1 apparently did not return to the first trailer but was looking around the tire of the trailer on which he was working when the lashing pulled tight and then through the bumper on the trailer. The chain backlashed, striking Employee #1 in the temple. He was killed.
