Employee's wrist fractured in fall from top of stepladder

Employee #1 was standing on the top cap of a stepladder to fill the water tank on top of a 14 ft high refrigerated trailer. The stepladder became unbalanced and tipped over, causing Employee #1 to fall 8 ft onto the pavement. He sustained a fracture left wrist.
