Employee crushed and killed when tractor overturns

At approximately 12:30 p.m. on October 8, 1999, Employee #1 was operating a Case 685 International tractor with a loader bucket on the front. He was piling and compacting corn silage into a 30 by 80 ft mound for seasonal storage; this involved running the tractor up and back along a 40 ft long ramp that ran from 0 to 10 ft in elevation. The tractor came too close to the left east edge of the mound, and the silage gave way under the tractor's wheels. The tractor overturned, crushing Employee #1. He was killed. The tractor was equipped with a seatbelt but not a roll bar.
