Investigation Summary
At 6:10 a.m. on April 5, 2001, Employee #1 was cleaning wood debris from beneath a chipper conveyor belt. Employee #1 was using an 11 foot aluminum pole rake to remove the debris. The end of the pole rake was pulled between an idler roller and the conveyor belt, causing Employee #1 to be fatally pinned between the pole and the conveyor belt.
