Investigation Summary
At approximately 7:45 a.m. on June 22, 2000, Employee #1 was transporting a load of PVC pipe from the J-M Manufacturing Company, Inc., of Wharton, TX, to the Coburn Supply Company, Inc., in Eunice, LA. The flatbed truckload consisted of 196, 20 ft lengths of 6 in. diameter PVC pipe, weighing 13,481.27 lb, and 56, 20 ft lengths of 12 in. diameter PVC pipe, weighing 10,483.20 lb. Employee #1 removed the straps from the cargo on the driver's side and had moved to the passenger's side of the trailer to pull the straps away there. A Coburn forklift operator had begun off-loading the 12 in. pipe from the driver's side of the trailer when the load shifted, causing the pipes to fall off the passenger's side. A Coburn foreman yelled at Employee #1 to move, but he was unable to get out of the way in time. Employee #1 was crouched over, tending to a ratchet strap, when two bundles of 6 in. pipe, weighing 1,299 lb, fell on him. He suffered massive head injuries, from which he died.
