Investigation Summary
Approximately 9:30 a.m. on August 10, 2000, Employee #1, a staff engineer for Geolabs Incorporated, fractured his pelvic bone when he was crushed by a rotating crane. Improvements were being made at the Waipahu Wastewater Pump Station to increase its capacity. RCI Construction Group Pacific was the general contractor. Geolabs Incorporated was the subcontractor for the pile driving quality control. Healy Tibbitts Builders Incorporated was the subcontractor for the pile driving. Employee #1 was at the Waipahu Wastewater Pump Station to observe the pile driving operations for quality control. He decided to stand next to the splice can pile at the front, right side corner which was the side closest to the feeder crane's blind side. The feeder crane operator did not see Employee #1 next to the splice can pile nor next to the feeder crane. The crane operator moved the crane back and rotated the crane in a clockwise direction. Employee #1 was caught in between the top portion of the splice can pile and the lower portion of the crane's rear counterweights. Employee #1 fractured his pelvic bone. The crane operation area and swing radius was not barricaded. He went to a local hospital for treatment.
