Employee injured in fall when concrete block breaks apart

Employee #1 and a coworker were standing on the leading edge of a 4 ft wide by 8 ft long by 1 ft thick piece of scored concrete when it cracked and broke off. The two workers fell 9 ft 6 in. Employee #1 sustained injuries and was out of work for approximately two weeks; the coworker was not injured.
