Employee dies when caught in between running machinery

On August 18, 1999, Employee #1 was operating the banding-packaging machine when his arm became caught between the lifting mechanism and the conveyor belt of the machine. He was unable to free his arm as the forklift portion of the machine continued to move forward puncturing the axilla of his right arm. He died as a result of the injuries sustained in the accident.
