Investigation Summary
A power line worker was working at the top of a 15.2-meter-tall utility pole. The employee, who was belted to the pole, was disconnecting the two conductors supported by the pole. When he disconnected them, the pole fell. The pole landed on the employee, killing him. The wooden pole, which was set to a depth of only 460 to 480 millimeters, was unable to sustain the employee's weight. The soil was soft, moist sand located in a low-lying area within 15 meters of a river. For all practical purposes, only the two attached conductors had supported the pole.
