Investigation Summary
Employee #1, a welder, mounted a scaffold through the fifth floor window opening, preparing to weld a the brick shelf at the same level. While his two helpers were at other locations, Employee #1 stood on the outriggers of the work platform and attempted to install work boards by removing boards from the permanent section of the platform and placing them where he needed them. As he was working, Employee #1's foot slipped off the scaffold outrigger and he fell approximately 65 ft to the ground. Employee #1 was killed. He was not using any form of fall protection.
