Investigation Summary
Employee #1 was killed when he and several other people were setting up a firework display in a fenced area. Employee #1 was burned when the fireworks they were setting up exploded. He later died. Two other people were seriously injured. Additionally, three adults and one juvenile managed to escape the area with minor injuries. Employee #1 went to a local hospital for treatments.
