Investigation Summary
A longshoreman was loading cargo in the hold of a ship. The hold level was about to be filled. The corner of the descending load partially landed on a stack of previously stowed containers that was located at the bow end of the hold. There was no one watching the load or warning employees of the descending load. The crane operator did not have a clear view of the hold deck or landing zone and the load continued to descend after it had become caught on the stowed containers. The corner of the load slid off of the containers it had been resting on. The freed load swung to the stern end of the hold and crushed the employee between the load and the stowed cargo. The employee died of internal injuries to the upper body that were sustained as a result of the accident.
