Investigation Summary
Employee #1 was assigned to relay hand signals from his supervisor to an equipment operator. The equipment operator was applying tension to a line which ran from his bobcat through a pulley mounted on a truck to a tree which was being cut down. The line failed, causing the pulley to recoil into Employee #1, striking his right thigh and causing a fractured femur and serious lacerations. Employee #1 was hospitalized for treatment and released.
