Investigation Summary
At approximately 1:00 p.m. on September 21, 1999, Employee #1 was spray painting a residence while standing on a 12 ft portable ladder. The slope of the wood-shingled roof was 22 degrees, and the feet of the ladder were on a slippery plastic drop cloth. As he was working, the feet of the ladder slid down the roof. Employee #1 fell off the ladder onto the roof, and then 16 ft 3 in. from the roof onto a concrete driveway. He suffered a fractured skull, a lacerated liver, and cerebral contusions and leg contusions. Employee #1 was pronounced dead at the scene. He was not using any fall protection and the ladder was not secured.
