Employee crushed and killed by falling backhoe

Employee #1, a foreman, was in an excavation, directing a backhoe operator as he was removing soil from the side of a concrete vault, so that a 4 in. water line could be connected. The excavation wall collapsed from under the backhoe, which fell into the excavation and onto Employee #1. He was crushed and killed.
