Employee fell over 9 ft to concrete floor and died

On the morning of March 17, 2000, a small boom truck arrived at a new single-family dwelling under construction. A crane operator set up his crane under the direction of Employee #1. The task at hand was to use the boom truck crane to place floor joists on the top plate of the first-story wall. The crane picked up the first set of floor joists and began raising them over the top horizontal wall plates approximately 8 ft in the air. Employee #1 used his hand level in an attempt to rotate the floor joists into position prior to placing them on the top of the framed wall. While looking up, Employee #1 walked into an unguarded stairwell opening and fell to the concrete floor basement below. The fall was approximately 9 ft 4 in. Employee #1 died the following morning as the result of head trauma.
