Investigation Summary
At approximately 9:45 a.m. on September 6, 1999, Employee #1 was operating a forklift, and drove off the end of a 4-ft-high dock. It was reported that his legs were pinned under the forklift. He died from his injuries a short time later at or on the way to a local hospital. Apparently, there was nothing mechanically wrong with the forklift, and the operator did not have an obstructed view of his path of travel. The forklift did not have a seatbelt.
