Employee Is Killed When Forklift Rolls Over

On December 28, 2000, Employee #1, a powered industrial truck operator, struck a curb outside a warehouse causing the forklift to roll onto his right side. The frame of the forklift cage came to rest on top of Employee #1, who was killed.
