Employee injured in fall into cyclone shaft

Employee #1 was part of a twelve-man crew of boiler makers at a power plant under construction. They were installing expansion joints in an outlet duct from the main boiler to the cyclone shaft. Employee #1 entered the duct through a 2 ft wide access door and fell 85 ft into the shaft. He suffered multiple fractures and was taken by ambulance to Allegheny County Hospital; after his condition stabilized, he was transferred to the Baltimore Shock Trauma Unit. The guardrails at the shaft had been removed to install insulation and had not been replaced.
