Investigation Summary
At approximately 7:00 p.m. on December 2, 1999, Employee #1 was feeding veneer into dryer Number 4 while working at plant Number 2. Other employees discovered Employee #1 with his chest pinned and crushed between the accumulator roller and the in-feed chain. Employees removed him and provided first aid until the ambulance arrived. Employee #1 was transported to a local hospital where he was pronounced dead upon arrival.
