Employee Killed When Crushed Between Mast And Cage

Employee #1 died when he was crushed between a mast and cage. He was in the process of unloading empty buckets off a truck using a fork lift when he decided to climb up onto the mast area. Employee #1 apparently hit the controls with his foot causing the mast to tilt and trapping him between the mast and the cage killing him.
