Investigation Summary
On July 24, 1999, Employee #1 was baling and hauling hay on a farm near Smiths Bend, TX, when a bale of hay fell on top of him. He went to Clifton Medical Clinic and Goodall/Witcher Hospital in Clifton, TX, where he was treated and released, although his right chest wall was bothering him. At approximately 12:00 midnight on July 26, 1999, he returned to the clinic. He was resuscitated briefly, but died later in the hospital.
