Investigation Summary

Investigation Nr: 951848
Event: 04/15/1999
Employee killed in fall from roof

At approximately 11:00 a.m. on April 15, 1999, Employee #1 and a helper were installing windows and doors on the interior of the future community center building under construction for a townhouse and condominium complex. Employee #1 climbed a wooden ladder that he had constructed earlier that day to gain access to the roof of the building. While he was doing this, his helper left to get gas for their boss's pickup truck. Employee #1 walked directly to the ridge of the roof and onto the back section. He then either slipped and lost his balance or the felt roofing paper tore as he walked on it, and he fell 19 ft onto the debris-covered ground below the eave of the roof. His head struck a small pile of hardened concrete left from pouring the swimming pool behind. Employee #1 was found 7.5 ft from the back wall of the community center and 2 ft from the swimming pool. He was pronounced dead on the scene by the attending paramedics.
