Investigation Summary
At approximately 4:00 p.m. on November 3, 1999, Employee #1 caught his left hand in the squeeze-press of a finger-joint machine. Employee #1 was in the process of clearing a jam and accidentally caught his hand in the squeeze-press return. The finger-joint machine was not locked out properly. Employee #1 sustained partial amputation of the middle finger on his left hand and the tips of the other three fingers were broken. Employee #1 was hospitalized.
