Employee injured in fall through metal deck

Employee #1 stepped out onto a 4 ft by 4 ft metal deck and fell through it, dropping approximately 29 ft onto a concrete floor. He sustained injuries that required hospitalization. Apparently, coworkers had cut the metal deck to install a curb and foundation for a gravity relief hood. They cut two parallel sides and then left the deck unsupported, unguarded, unattended, and unbarricaded.
