Investigation Summary

Employee #1 was part of a four-man steel erection crew that was erecting roof joist. The joists were 22 ft above floor grade and the joist span was less than 40 ft. The joist did not have apparent trip hazards such as clips, holes, or surface coatings. At the time of the accident the joist were welded on both ends. Employee #1 was assigned the task of shaking out the steel and threading the bridging. The employee was preparing to thread the bridging. He lost his balance, fell from the joist, and landed on the ground 22 ft below, where he died.
