Investigation Summary

Investigation Nr: 201951456
Event: 04/09/1999
Employee foot amputated when crushed by forklift

On April 9, 1999, a temporary employee was instructed to operate a forklift truck known as the "Prime mover". The employee had not received training in how to operate the forklift. While operating the forklift, it picked up speed, and the employee attempted to jump off of the back of the truck. The employee was not able to get his foot out before it became caught between the rack and the forklift. Due to an infection that set in, the employee received a partial foot amputation, 60 percent, of his right foot.
