Investigation Summary
Employee #1 was repairing a leak in an air line located approximately 18 inches below the ceiling, and approximately 15 feet above the floor. To gain access to the air line, a pallet was placed on the forks of a fork lift truck, and Employee #1 was elevated to reach the air line. The pallet used was a new pallet, but it had four spaces between the boards, ranging from 3.5 inches to 5.5 inches in width. Employee #1 fell from the pallet, landing on his head and shoulder. There were no witnesses to the fall. Two of the boards had small broken areas on the edge, indicating that it is likely Employee #1 stepped between the boards and lost his balance. Employee #1 died of his head injuries the following day.
