Investigation Summary
At approximately 8:00 a.m. on December 1, 1998, Employee #1 was working from the flat roof of a building. He fell approximately 7 ft, striking his head on a company truck and then on the concrete pavement. Employee #1 was airlifted to Norfolk Sentara Hospital, where he died on December 7, 1998, of head injuries sustained in the accident.
