Investigation Summary
On April 28, 1999, Employee #1 was working from a 6 ft Baker scaffold. He was using a 6 ft A-frame ladder atop the scaffold to reach the ceiling when he lost his balance and fell approximately 12 ft. He sustained multiple severe head injuries and was hospitalized for about three weeks. He was readmitted about two weeks later, and remained hospitalized until his death on June 6, 1999.
