Investigation Summary

Investigation Nr: 201630779
Event: 03/02/1999
Employee injured in fall from roof

On March 2, 1999, Employee #1 was filling flower beds with top soil while another employee, the foreman, was on the roof laying tar paper. When the other employee needed more tar paper he asked Employee #1 to bring him some. When Employee #1 got back with the load of tar paper, he climbed the ladder and stepped off onto a roof with an approximate pitch of 3 by 12. Since Employee #1's boots were covered in top soil, as soon as he put his weight on the roof he slid off and landed on the ground. Employee #1 fell approximately 10 feet 4 inches. The employee was transported to a hospital, treated for a dislocated shoulder, and kept overnight.
