Investigation Summary
At approximately 12:53 p.m. on November 2, 1998, Employee #1 and coworkers were using a forklift to transport mat grids of reinforcing steel rods from a staging area to the rear of a crane, where they would be hoisted and set in place. The forklift boom and forks were extended approximately 25 ft upward to accommodate lifting the large mats. At the time of the accident, the forks were under only one of the intertwined reinforcing rods. During transport, Employee #1 held onto the side of the mat to help prevent it from swinging. As they were moving, one side of the mat broke loose. The forklift driver yelled to Employee #1 to look out. He was trying to lower the mat when it collapsed and fell on Employee #1, striking him on the side of the head. Emergency Services was called and he was flown by helicopter to the hospital, where he was pronounced dead in the emergency room.
