Employee killed in fall from steel construction

Employee #1 was working approximately 28 ft above the lower level of a building, making a double connection and removing the last two bolts from an existing beam. He was using a body harness and a lanyard. Employee #1 was bending over to remove the two bolts when he rolled out of his lanyard and fell onto a concrete pad. He was killed. The lanyard's self-closing latch had remained fully open,
