Employee killed in fall from stepladder

Employee #1 was working from a 6 ft tall stepladder that was resting on a 19 in. high by 32 in. wide by 64 in. long platform. He fell from the ladder and off the platform, striking his head on the concrete floor as he landed. Employee #1 was killed.
