Investigation Summary
At 12:30 p.m. on May 6,1999, Employee #1's coworker was using a Toyota forklift to load pallets of concrete blocks onto trailer trucks. The forklift malfunctioned and the forks became stuck in the raised position. Employee #1, the plant superintendent, came out to the yard to inspect the lift truck. He was walking under the forks when they suddenly fell, descending on his head. He was crushed and killed.
