Investigation Summary
Employee #1 had received his morning work assignment from his supervisor and went to work in the compost yard. Somehow during the course of his work operation he was transported through a transfer conveyor belt system on a compost debagger machine. Employee #1 was dumped out the end of the conveyor belt system, falling 12 feet face-first to the asphalt floor of the yard below the conveyor belt, sustaining fatal injuries.
