Investigation Summary
Employee #1, a packer for a company that packaged dry dog food, was performing his normal job duties, when he found that the dry dog food had become moist and was sticking to side of the bin. The storage bin had an 18 ft tall straight section and an 8 ft cone-shaped section. Employee #1 entered the bin and was using a pick to try to dislodge the stuck dog food. As he was working, the dog food came loose and fell onto Employee #1, engulfing him. He died of suffocation. The company later estimated that Employee #1 had been buried under 1 ton of dog food.
