Investigation Summary
On October 27, 1998, an employee reached into the point of operation and used her hand to remove slugs under the die area. The employee did not use a rake to remove slugs and she was not adequately trained. The point of operation was not adequately guarded; it had a Link Lite II sensing device at the front only and no side guard. The sensing device was muted and had a gap. The employee got her hand caught in the die area and amputated tip of her thumb, first and ring finger. The employee was hospitalized and treated for amputations of fingers.
