Employee killed in fall from balcony

Employee #1 was hired on May 25, 2000, as a carpenter at the construction site of a new apartment building. He worked two hours that day installing decking, posts, and guardrails for some new balconies. Employee #1 returned on May 30, 2000, and worked for several hours decking the 6 ft by 12 ft balcony of apartment #231. He laid out some 5 1/2 in. wide by 6 ft long by 1 in. thick wooden boards on top of five wooden runner support beams, and was nailing the boards to the support beams when he fell 19 ft from the balcony to the ground. Employee #1 was killed. There were no guardrails on the balcony, nor was Employee #1 using a personal fall arrest system or a safety net system.
