Investigation Summary
On January 9, 2000, Employee #1 and two coworkers were at an oil well drilling site, laying down drilling pipe. The crew and their equipment had arrived at the site at 3:00 p.m. and had begun laying down 9,650 ft of drilling pipe in 31 ft units. By 11:15 p.m., approximately 85 lengths of the pipe had been removed from the well and stacked four rows high, pipe on pipe. The stacked pipes unexpectedly shifted approximately 15 ft and fell on top of Employee #1. He suffered multiple injuries, from which he died.
