Investigation Summary
Employee #1 pulled out a telephone line from the reel and laid it alongside the roadway. He turned and was walking away when a truck came down the highway and struck the cable. It became caught on the truck and was pulled down the highway, dragging the reel behind it. The reel struck Employee #1 on the head, killing him.
