Investigation Summary

Investigation Nr: 202043923
Event: 10/11/1999
Employee's tractor with brush hog rollover, causing death

On October 11, 1999, Employee #1, employee for Proctor Maple Research Center, was using a Ford Model 1500 compact tractor, with a rear PTO driven brush hog, to mow a field. The field was on a slope with a rutted trail/road running through it. Employee #1 was mowing on the lower side of this road immediately adjacent to the edge of the road when the tractor apparently rolled over, ejecting him from the seat and the brush hog came in contact with his upper extremities, causing his death. There were no witnesses and he was not found until approximately 6:45 p.m. that evening.
