Employee Killed When Struck By Steel Tank

A coworker was using a jib-hoist to relieve tension on the debris tank tailgate at the rear of the tank. Employee #1 was at the front of the tank cleaning weld splatters with a hand grinder. The tension on the tailgate caused the tank to tilt and roll off of its stands causing Employee #1 to be struck by the tank ductwork. Employee #1 was killed.
