Investigation Summary
On November 16, 1999, Employee #1 was walking on the first floor of a new house under construction when he stepped upon a sheet of plywood covering a basement stairwell opening. The force of his mass displaced the plywood and he fell 8 ft onto the basement floor where he struck the back of his head. The impact of the fall caused him to receive traumatic brain injures resulting in the loss of consciousness. He was transported to the hospital but later died from his injures sustained in the accident.
