Employee's hand fractured in pin assembly machine

Employee #1 was operating a custom-made electro-pneumatic pin assembly machine with automatic feed to assemble 4 1/2 in. long by 1 1/2 in. wide shelf collars. The machine jammed and she was reaching into the point of operation to clear it when the machine cycled. Employee#1's right hand became trapped and she sustained multiple lacerations and fractures. The employer had a safety program, but had not addressed hazardous energy control procedures for the machine. The machine's point of operation was not guarded and procedures were not in place for clearing jams.
