Event: 10/16/1998
Five employees injured when concrete form collapses

At approximately 2:00 p.m. on October 16, 1998, Employee #1, a carpenter, was monitoring a concrete pour operation under the roof forms for a underground mechanical room. Employees #2 through #5, all concrete finishers, were pouring the concrete and undertaking finishing operations on top of the newly laid roof deck. The shoring for the forms suddenly collapsed, and Employee #1 was partially buried under them, the roof decking, the reinforcement steel bars, and the freshly poured concrete. He suffered a fractured back, for which he was hospitalized. Employees #2 through #5 rode the falling load to the ground; all sustained multiple contusions, for which they were treated and released.
