Investigation Summary
Employee #1 suffered a bruise on his head and strained ligaments in his left elbow while he was operating a forklift, removing bales of wood pulp from a warehouse, and loading a truck trailer. The load was picked up from 10 feet high, and the operator failed to lower the load while he was backing out of the aisle. He hit the corner of the stack next to him and tipped the forklift over. As the forklift was tipping over, he managed to exit the forklift seat and roll before being hit by the load. He was treated at a local hospital.
