Employee killed in fall during roof decking operation

Employee #1 was on a crew installing steel decking panels on the roof of a church parish hall as part of an expansion project. He was working directly over the open stairwell area when he fell 34.8 ft through it. No intermediate flooring had been installed and Employee #1 managed to grab a steel beam as he fell but was unable to maintain his grip. He struck his head on a lower beam as he dropped and flipped over before landing on the foundation slab. Employee #1 was transported to the hospital, where he died two days later. The workers on this decking operation were not protected by any means of fall protection or by personal fall arrest equipment.
