Investigation Summary
After sundown, Employee #1, an 18-year-old seasonal worker, was in a cotton bale storage warehouse, flagging and identifying the bales. The cotton bales measured roughly 5 ft by 3 ft by 2 ft and each weighed about 500 lb. For some reason, Employee #1 was climbing on a stack of two or three bales when the stack shifted and the bales toppled over. He fell onto the concrete floor and was killed. Subsequent investigation revealed that the stacking, which was done using a powered industrial trucks, was irregular and unstable at some points. Some of the narrow aisles had also become partially blocked by leaning bales. The lighting inside the building had deteriorated during the high-volume season, and it appeared questionable the extent to which newly-hired workers were aware of the hazards involved with working on and around the warehouse stack.
